Athletics Clearance

Dear Parents and Guardians,
We are partnering with Exer Urgent Care, and they are providing complimentary sports screenings for our athletes through their Exer Sports & Schools Program!

To take advantage of this opportunity:
➢ Sign up for an appointment
    You must make an appointment in advance using the the link or QR Code below.
    Exer cannot accommodate walk-ins for these complimentary sports screenings.
    If there are multiple student athletes in the same family, please make an individual appointment for each athlete.
➢ The athlete must bring a copy of our school sports screening form.
➢ A parent or guardian must accompany any student athlete through the screening exam.
➢ If your wear glasses (or contacts) please ensure you bring them to the appointment.
➢ The student athlete will be provided the completed sports screening form and is responsible for submitting the form to our athletic department (Exer is not responsible for submitting any forms).
➢ In consideration of other families, please be prompt for your appointment.

The physical should take approximately 15 minutes but there may be a wait depending on flow of business and patient arrival times.
